Community Health Needs Assessment (CHNA)

A Community Health Needs Assessment (CHNA) is a systematic, data-driven approach to determining the health status, behaviors and needs of residents in the service area of Hackensack Meridian Health and its hospitals. The Hackensack Meridian Health Network completed CHNAs for each hospital facility.  All reports have been formally approved and adopted as required by IRS regulations.
