COVID-19 Resources for Primary Care
Having a primary care physician or advanced practice provider is essential to your overall health and wellness. In addition to being a resource when you are not feeling well and helping you to manage chronic conditions, they are the professionals who, through your wellness exam, will help to identify preventative measures needed to live the active lifestyle you desire.
A wellness appointment is an opportunity for you and your provider to establish open communication and will typically include the following:
- Review of your health history
- A physical exam
- Discussion on any physical or mental concerns
- Review medications
- Administer or prescribe recommended screening and immunizations
- Collaboratively establish a plan for your health and wellbeing
Our Primary Care Practitioners focus on:
- Preventative health
- Acute and chronic conditions
- Geriatrics
- Cardiovascular health
- Women’s health

Many of our Hackensack Meridian Health providers have been recognized by the National Committee for Quality Assurance (NCQA) for the high-quality care they provide to diabetic and heart/stroke patients, and many of our practices have achieved the status of Patient-Center Medical Home (PCHM).
