Medical exams and mental health screenings for infants to 18-year-olds.
The Comprehensive Health Evaluations for Children (CHEC) Program is designed to provide comprehensive health evaluations of children residing in out-of-home placements upon referral from the State of New Jersey’s Department of Children and Families Division of Child Protection and Permanency (DCPP). Comprehensive evaluations include medical and developmental/mental health for children in foster care in the following 5 northern counties of New Jersey: Bergen, Passaic, Hudson, Morris and Sussex.
Our Services:
- Well child medical exams:
- Vision and hearing screenings
- Immunizations/vaccine information statements/ NJIIS Vaccine Registary entry and record
- Age-specific educational information
- Laboratory tests
- EPSDT (Early and Periodic Diagnosis and Treatment services) as recommended by the American Academy of Pediatrics
- Age-appropriate mental health screenings:
- Developmental assessments
- Cognitive screenings
- Psychological and behavioral questionnaires and interview of child with collateral information from caseworker/parent/resource parent as available
- A medical and mental health preliminary report of both evaluations is provided at the conclusion of the visit with a final report including all results and recommendations completed and shared within 2 weeks
