The Subacute Care Unit at Meridian Nursing & Rehab at Brick cares for patients who require complex medical care, skilled nursing and progressive rehabilitation services.

Subacute Care


Subacute care assists those who have experienced catastrophic trauma, illness, or surgery with their recuperation, helping them function better, sooner.

Using an interdisciplinary plan of skilled care and rehabilitation, our medical staff delivers subacute care for:

Heart Care

At Hackensack Meridian Health Nursing and Rehab at Brick, we are proud to be the only skilled nursing facility in New Jersey to receive the Advanced Certification in Heart Failure from the Joint Commission in conjunction with the American Heart Association. The Joint Commission accredits and certifies more than 20,000 health care organizations and programs in the United States. Joint Commission accreditation and certification is recognized nationwide as a symbol of quality.

The Joint Commission’s Advanced Certification in Heart Failure Program, launched in 2011, is designed to target methods of providing safe, successful transitions of care as the patient moves from the inpatient setting to an outpatient setting. Achievement of this certification signifies our dedication and commitment to the achievement of long-term success in improving outcomes for patients diagnosed with and being treated for heart failure.

Hospice Care

If your loved one should develop end-of-life needs, there’s no reason to move him or her to another facility. We offer hospice care right on site, providing compassionate and high-level medical care.

Dementia Unit

We recognize the special needs of people with Alzheimer’s and other memory-related diseases. That’s why our residents with dementia live in a specially secured environment within our facility and receive specialized care from team members that are Certified Dementia Practitioners.
