Nondiscrimination Policy:

As a recipient of funding through Federal Health Care programs, Meridian Health does not exclude, deny benefits to, or otherwise discriminate against any person on the ground of race, color, or national origin, sexual orientation, gender identity and/or expression, veteran status, on the basis of disability or age, in sum or substance, 'any other category protected by state or federal law’, in admission to, participation in, or receipt of services and benefits under any of its programs and activities. This statement is in accordance with the provisions of Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973, the Age Discrimination Act of 1975, and Section 1557 of the Affordable Care Act (ACA) of 2010 (42 U.S.C. 18116). Translation Policy:

Meridian at Home and Meridian Hospice provide free aids and services to people with disabilities to communicate effectively with us, such as:

  • Qualified sign language interpreters
  • Written information in other formats (large print, audio, other formats)
  • We can also provide free language services to people whose primary language is not English, such as qualified interpreters
  • Information written in other languages

If you need these services, please contact Karen Stanton, Civil Rights Coordinator. If you believe that Meridian at Home or Hospice has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability or sex or other category in our Nondiscrimination Policy, you can file a grievance with :
