Meridian At Home offers a variety of rewarding career opportunities, including: 


Registered Nurses (RN)           

Responsible for rendering professional nursing care to patients in their home by assessing, developing, implementing and evaluating home nursing needs on individual and continuing basis of assigned patients in keeping with agency policies & procedures and external regulations.


Physical Therapists (PT)           

Responsible for rendering professional rehabilitation therapy to patients in their homes by evaluating, developing and implementing individual goal oriented therapy programs in keeping with agency policies and procedures and external organizations.


Occupational Therapists (OT)           

Responsible for rendering professional rehabilitation therapy to patients in their homes by evaluating, developing and implementing individual goal oriented therapy programs in keeping with agency policies and procedures and external organizations.


Speech Therapists (ST)           

Responsible for rendering professional rehabilitation therapy to patients in their homes by evaluating, developing and implementing individual goal oriented therapy programs in keeping with agency policies and procedures and external organizations.


Social Workers (LCSW)             

Responsible for rendering professional social work services to patients in their home by assessing, developing, implementing and evaluating the social and emotional needs of patients in accordance with agency policies and procedures applicable to social work standards and governmental laws and requirements within the scope of medical home care.


Licensed Practical Nurses (LPN)           

Renders nursing care to patients in their homes by assessing, developing, implementing and evaluating, home nursing care needs of assigned patients under the direction of a registered nurse.


Certified Home Health Aides (CHHA)           

Under the direction of the RN, provides in‐home, para‐professional services necessary for caring for the patient's personal needs.


Chaplains           

Responsible for the provision of spiritual support to patients and families in keeping with the patient/family's belief system. Responsible for bereavement follow‐up as assigned by supervisor.
